Contact Form Issues

1. My "Get Appointment" Submit button does not respond when I click on it on the pop-up form. Although the Testimonial form works perfectly. And when I checked from your Demo version "Get Appointment" submit does not respond either.

2. When you click on "Get Appointment" using a phone the menu bar shows up and covers some part of the form

Seems like this form needs to be regenerated please send me your WordPress logins i will check if your form is built correctly thanks.
