Make 2 boxes - Med Access box

Can you make the Pricing Box on the left that says Med Access have 2 boxes inside to add the text? After the pricing of $14.99, there is text you can add, I need 2 boxes. 

Look at the image above as an example. I added an image above for you to see. It says Med Access and then it says 24/7 Telemedicine and Discoutn Card, see on the bottom there are 2 boxes. 

I have created two boxes under Med Access pricing table which you can further edit with your own text. If there is any problem let me know i will handle that onward.
