There is a new shortcode for Booking page which let your customers select brand, device, service give their information and book their service please try that shortcode.

And in case you want to edit the request quote form or page, you would have to do that yourself and you should know some PHP to do this. The file to edit request quote form is >> In plugin >> lib >> includes >> shortcodes >> request_quote.php
That is the file you have to edit to customize the request quote form.
